    Phase diagrams of a p-Wave superconductor inside a mesoscopic disc-shaped sample

    We study the finite-size and boundary effects on a time-reversal-symmetry breaking p-wave superconducting state in a mesoscopic disc geometry using Ginzburg-Landau theory. We show that, for a large parameter range, the system exhibits multiple phase transitions. The superconducting transition from the normal state can also be reentrant as a function of external magnetic field.Comment: Revised version published in Physical Review

    Lipidemia status among HIV positive adult male on HAART attending the HIV clinic at Kericho district hospital, Kericho

    Background: It is unfortunate that while Highly Active Anti-retroviral Therapy (HAART) has become the standard of care among HIV positive patients, the medications have been associated with metabolic abnormalities recognised to cause lipidemia.Objective: To establish lipidemia status among HIV positive adult male patients attending HIV clinic at Kericho District Hospital.Design: A cross sectional analytical study.Setting: HIV clinic at Kericho District Hospital, Kenya.Subjects: HIV positive adult male patientsResults: The results indicated that the mean age was 43.52 ± 9.17 years and out of which 82.27% were married. Primary Education level attained 40.97% .The mean lipid profile level was LDL 2.5 ± 1.05mmol/l, cholesterol 4.49 mmol/L ± 1.28 mmol/l, HDL 1.47 ± 0.58 mmol/l and triglycerides 1.96 ± 1.32 mmol/l, against the expected level of LDL (1.1. - 2.4 mmol/L), HDL (0.9 - 1.68 mmol/L) triglyceride (0.41 - 2.61 mmol/L) and total cholesterol (2.55 - 5.7 mmol/L).The prevalence lipidemia was 48.17%.Conclusion: The findings showed that the mean LDL was elevated with proportion of lipidemia at a significant higher level among HIV positive adult patients on HAART with considerable improvement in the nutritional status. Future work should investigate the biological mechanisms and pathways through which micronutrients affects high density lipoprotein (HDL) and low density lipoproteins (LDL)

    Background: Nutrition and HIV are strongly related and any immune impairment as a result of HIV leads to malnutrition while malnutrition leads to immune impairment and thus worsens the effect of HIV. Individuals living with HIV have special nutritional needs irrespective of whether they are on anti-retroviral treatment or receiving care services.Objective: To establish nutritional status of adult male on ART at Kericho District Hospital, Kenya.Design: Cross sectional analytical study.Setting: HIV clinic at Kericho District Hospital, Kenya.Subjects: HIV positive adult male patientsResults: Body Mass Index ≥18.5 and ≤ 24.5 (70.92), >16 & < 18.5 (11.94), >25.5 and < 30 (11.94), ≤ 16 (2.58) and ≥30.Conclusion: There is a high burden of malnutrition among PLHIV on ART (29.1%). Nutritional assessment, Counseling and support (NACS) should be the key intervention during the course of chronic HIV care and follow up

    Adaptive working memory strategy training in early Alzheimer's disease: randomised controlled trial.

    BACKGROUND: Interventions that improve cognitive function in Alzheimer's disease are urgently required. AIMS: To assess whether a novel cognitive training paradigm based on 'chunking' improves working memory and general cognitive function, and is associated with reorganisation of functional activity in prefrontal and parietal cortices (trial registration: ISRCTN43007027). METHOD: Thirty patients with mild Alzheimer's disease were randomly allocated to receive 18 sessions of 30 min of either adaptive chunking training or an active control intervention over approximately 8 weeks. Pre- and post-intervention functional magnetic resonance imaging (fMRI) scans were also conducted. RESULTS: Adaptive chunking training led to significant improvements in verbal working memory and untrained clinical measures of general cognitive function. Further, fMRI revealed a bilateral reduction in task-related lateral prefrontal and parietal cortex activation in the training group compared with controls. CONCLUSIONS: Chunking-based cognitive training is a simple and potentially scalable intervention to improve cognitive function in early Alzheimer's disease

    The importance of sustained attention in early Alzheimer's disease

    INTRODUCTION: There is conflicting evidence regarding impairment of sustained attention in early Alzheimer's disease (AD). We examine whether sustained attention is impaired and predicts deficits in other cognitive domains in early AD. METHODS: Fifty-one patients with early AD (MMSE > 18) and 15 healthy elderly controls were recruited. The sustained attention to response task (SART) was used to assess sustained attention. A subset of 25 patients also performed tasks assessing general cognitive function (ADAS-Cog), episodic memory (Logical memory scale, Paired Associates Learning), executive function (verbal fluency, grammatical reasoning) and working memory (digit and spatial span). RESULTS: AD patients were significantly impaired on the SART compared to healthy controls (total error β = 19.75, p = 0.027). SART errors significantly correlated with MMSE score (Spearman's rho = -0.338, p = 0.015) and significantly predicted deficits in ADAS-Cog (β = 0.14, p = 0.004). DISCUSSIONS: Patients with early AD have significant deficits in sustained attention, as measured using the SART. This may impair performance on general cognitive testing, and therefore should be taken into account during clinical assessment, and everyday management of individuals with early AD

    Blue reflectance in tarantulas is evolutionarily conserved despite nanostructural diversity

    Slight shifts in arrangement within biological photonic nanostructures can produce large color differences, and sexual selection often leads to high color diversity in clades with structural colors. We use phylogenetic reconstruction, electron microscopy, spectrophotometry, and opticalmodeling to showan opposing pattern of nanostructural diversification accompanied by unusual conservation of blue color in tarantulas (Araneae: Theraphosidae). In contrast to other clades, blue coloration in phylogenetically distant tarantulas peaks within a narrow 20-nm region around 450 nm. Both quasi-ordered and multilayer nanostructures found in different tarantulas produce this blue color. Thus, even within monophyletic lineages, tarantulas have evolved strikingly similar blue coloration through divergent mechanisms. The poor color perception and lack of conspicuous display during courtship of tarantulas argue that these colors are not sexually selected. Therefore, our data contrast with sexual selection that typically produces a diverse array of colors with a single structuralmechanismby showing that natural selection on structural color in tarantulas resulted in convergence on similar color through diverse structural mechanisms

    Plasmon reflections by topological electronic boundaries in bilayer graphene

    Domain walls separating regions of AB and BA interlayer stacking in bilayer graphene have attracted attention as novel examples of structural solitons, topological electronic boundaries, and nanoscale plasmonic scatterers. We show that strong coupling of domain walls to surface plasmons observed in infrared nanoimaging experiments is due to topological chiral modes confined to the walls. The optical transitions among these chiral modes and the band continua enhance the local ac conductivity, which leads to plasmon reflection by the domain walls. The imaging reveals two kinds of plasmonic standing-wave interference patterns, which we attribute to shear and tensile domain walls. We compute the electronic structure of both wall varieties and show that the tensile wall contain additional confined bands which produce a structure-specific contrast of the local conductivity. The calculated plasmonic interference profiles are in quantitative agreement with our experiments.Comment: 14 pages, 5 figure

    Nkx2-5 and Sarcospan genetically interact in the development of the muscular ventricular septum of the heart

    The muscular ventricular septum separates the flow of oxygenated and de-oxygenated blood in air-breathing vertebrates. Defects within it, termed muscular ventricular septal defects (VSDs), are common, yet less is known about how they arise than rarer heart defects. Mutations of the cardiac transcription factor NKX2-5 cause cardiac malformations, including muscular VSDs. We describe here a genetic interaction between Nkx2-5 and Sarcospan (Sspn) that affects the risk of muscular VSD in mice. Sspn encodes a protein in the dystrophin-glycoprotein complex. Sspn knockout (Sspn(KO)) mice do not have heart defects, but Nkx2-5(+/−)/Sspn(KO) mutants have a higher incidence of muscular VSD than Nkx2-5(+/−) mice. Myofibers in the ventricular septum follow a stereotypical pattern that is disrupted around a muscular VSD. Subendocardial myofibers normally run in parallel along the left ventricular outflow tract, but in the Nkx2-5(+/−)/Sspn(KO) mutant they commonly deviate into the septum even in the absence of a muscular VSD. Thus, Nkx2-5 and Sspn act in a pathway that affects the alignment of myofibers during the development of the ventricular septum. The malalignment may be a consequence of a defect in the coalescence of trabeculae into the developing ventricular septum, which has been hypothesized to be the mechanistic basis of muscular VSDs

    Do HIV treatment eligibility expansions crowd out the sickest? Evidence from rural South Africa

    OBJECTIVE: The 2015 WHO recommendation to initiate all HIV patients on antiretroviral therapy (ART) at diagnosis could potentially overextend health systems and crowd out sicker patients, mitigating the policy's impact. We evaluate whether South Africa's prior eligibility expansion from CD4 ≤200 to CD4 ≤350 cells/μL reduced ART uptake in the sickest patients. METHODS: Using data on all patients presenting to the Hlabisa HIV Treatment and Care Program in KwaZulu-Natal from April 2010 - June 2012 (n=13,809), we assessed the impact of the August 2011 eligibility expansion on the number of patients seeking care, number initiating ART, and time from HIV diagnosis to ART initiation among patients always eligible (CD4 0-200), newly eligible (CD4 201-350), and not yet eligible by CD4 count (>350). We used interrupted time series methods to control for long-run trends and isolate the effect of the policy. RESULTS: Expanding ART eligibility led to an increased number of patients initiating ART per month [+95.5; 95% CI (-1.3; 192.3)]. Newly eligible patients (CD4 201-350) initiated treatment 47% faster than before (95% CI 19%; 82%), while the sickest patients (CD4 ≤200) saw no decline in the monthly number of patients initiating treatment or the rate of treatment uptake. CONCLUSION: The Hlabisa program successfully extended ART to patients with CD4 ≤350 cells/μL, while ensuring that the sickest patients did not experience delays in ART initiation. Treatment programs must be vigilant to maintain quality of care for the sickest as countries move to treat all patients irrespective of CD4 count. This article is protected by copyright.     Automaticity and localisation of concurrents predicts colour area activity in grapheme-colour synaesthesia

    In grapheme-colour synaesthesia(GCS), the presentation of letters or numbers induces an additional ‘concurrent’ experience of colour. Early functional MRI (fMRI) investigations of GCS reported activation in colour-selective area V4 during the concurrent experience. However, others have failed to replicate this key finding. We reasoned that individual differences in synaesthetic phenomenology might explain this inconsistency in the literature. To test this hypothesis, we examined fMRI BOLD responses in a group of grapheme-colour synaesthetes (n¼20) and matched controls(n¼20) while characterising the individual phenomenology of the synaesthetes along dimensions of ‘automaticity’ and ‘localisation'. We used an independent functional localiser to identify colour-selective areas in both groups. Activations in these areas were then assessed during achromatic synaesthesia-inducing, and non-inducing conditions; we also explored whole brain activations, where we sought to replicate the existing literature regarding synaesthesia effects. Controls showed no significant activations in the contrast of inducing > non-inducing synaesthetic stimuli, in colour-selective ROIs or at the whole brain level. In the synaesthete group, we correlated activation within colour-selective ROIs with individual differences in phenomenology using the Coloured Letters and Numbers (CLaN) questionnaire which measures,amongst other attributes, the subjective automaticity/attention in synaesthetic concurrents, and their spatial localisation. Supporting our hypothesis, we found significant correlations between individual measures of synaesthetic phenomenology and BOLD responses in colour-selective areas, when contrasting inducing- against non-inducing stimuli. Specifically, left-hemisphere colour area responses were stronger for synaesthetes scoring high on phenomenological localisation and automaticity/attention, while right-hemisphere colour area responses showed a relationship with localisation only. In exploratory whole brain analyses, the BOLD response within several other areas was also correlated with these phenomenological factors, including the intra parietalsulcus, insula, precentral and supplementary motor areas. Our findings reveal a network of regions underlying synaesthetic phenomenology and they help reconcile the diversity of previous results regarding colour-selective BOLD responses during synaesthesia, by establishing a bridge between neural responses and individual synaesthetic phenomenology
